Student Qualifies for services

• SIT/Screening Documentation (must include Hearing and Vision screening)

• Prior Written Notice for Evaluation Form   Contact your building psychologist Not for teacher use

• Notice of Meeting   This is on Web Kids

• Parental Rights

• Team Evaluation Summary and Eligibility (Report is optional) Not for teacher use   

  o     Must include all test information and scores

  o    If Team Evaluation Report is used in place of Team Evaluation Summary and Eligibility, you must at least add the agree/disagree page to the report and the report must address all eligibility issues. 

• Authorization Of Disclosure/Release of Information (for Doctors, Mental Health, outside family & agencies)

• IEP

• Prior Written Notice for Identification Form   Contact your building psychologist Not for teacher use 

• Make sure all paperwork is copied and given to parent, a copy is left for teacher file, and all originals are sent to the main office to the MIS clerk

Student DOES NOT Qualify for services
