• Teacher/Providers type in Teacher Information Page (and turn in to main office ASAP)

• Parental Rights

•  The team can choose to adopt current IEP or conduct an initial evaluation

• Meet with administrator, general education teacher, special education teacher and parent as soon as possible and complete Conference Summary    

  o      Acknowledge Special Education services on summary

  o      Accept current IEP or new IEP will be completed within (days) on summary

• Spectra forms for students receiving related services

  o      Parent Release of Information and Physician Authorization

  o      Student Information Form -enter, exit, exceptionality change 

• Make sure all paperwork is copied and given to parent, a copy is left for teacher file, and all originals are sent to the main office to the MIS clerk
