•  IEP changes - Amend the Teacher Information Page

• Prior Written Notice for Identification etc  Not for teacher use       Parent consent is required when more than 25% change in   time is occurring.

• Conference Summary                                                                       This should include justification of why time is changing.

• Make sure all paperwork is copied and given to parent, a copy is left for teacher file, and all originals are sent to the main office to the MIS clerk
